1.Acknowledgment of Risk
I understand that Brazilian Jiu-Jitsu (BJJ) and martial arts training involve physical contact and inherent risks, including but not limited to serious injury, permanent disability, or death. Risks may include falls, joint injuries, strains, sprains, cuts, bruises, and other physical harm.
I voluntarily choose to participate in training, classes, sparring, and related activities at Jones Family Jiu-Jitsu.
2. Release of Liability
I hereby release, waive, and discharge Jones Family Jiu-Jitsu, its owners, instructors, staff, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, damage, or loss that may occur as a result of my participation.
3. Assumption of Risk
I fully understand the risks involved and voluntarily assume all responsibility for any injuries or damages that may occur during participation.
4. Rules & Conduct
I agree to follow all gym rules and instructor directions. I understand that unsafe behavior may result in removal from training.
5. Photography & Media Release
I grant permission for Jones Family Jiu-Jitsu to use photos or videos of me taken during training for promotional purposes, unless I notify the gym in writing.

7. MEMBERSHIP & PAYMENT TERMS
Weekly Direct Debit
Gym membership fees are paid by weekly direct debit and continue on an ongoing basis until the membership is cancelled in accordance with the cancellation terms below.
By signing this waiver, the member or parent/guardian authorises Jones Family Jiu-Jitsu to process the agreed weekly membership payment on a continuous basis.
Class Cancellations
While Jones Family Jiu-Jitsu makes every effort to ensure scheduled classes run as advertised, occasionally a class may need to be cancelled due to instructor availability or unforeseen circumstances.
If an instructor is unable to attend and a scheduled class is cancelled, members are welcome to attend any of the other suitable classes offered by Jones Family Jiu-Jitsu during that same week, subject to class availability and age/skill requirements.
No refund or credit will ordinarily be provided for an individual class cancellation where alternative classes are available during the week.
Membership Holds / Pauses
A minimum of two (2) weeks’ notice is required for any request to place a membership on hold or pause.
Membership holds are subject to approval by Jones Family Jiu-Jitsu and must be requested in advance.
Membership Cancellations
A minimum of two (2) weeks’ notice in writing (Email) is required to cancel a membership.
Membership payments will continue during the required two-week notice period. Any outstanding fees must be paid before the membership can be considered fully cancelled.
Responsibility for Membership Details
Members or parents/guardians are responsible for ensuring that their direct debit/payment details remain current and that sufficient funds are available for scheduled membership payments. Any failed or dishonored payments may be subject to applicable fees.
I understand that Jones Family Jiu-Jitsu is not responsible for lost, stolen, or damaged personal items.
Lastly, the owner of the property located at 104 Tanner Street, Breakwater, accepts no responsibility or liability for the health, safety, or well-being of students attending Jones Family Jiu-Jitsu.
Students, parents, and visitors attending Jones Family Jiu-Jitsu are expected to conduct themselves respectfully at all times and must not obstruct, interfere with, or disturb any other tenants or businesses operating on the property. Access ways, shared areas, and parking spaces must be kept clear and used considerately
